Bioadaptive Stress Repair — BSR — is Pavise’s regenerative and reparative night therapy, described by the brand as a biointelligent catalyst for skin regeneration, repair, and stress defense, and positioned as the first daily topical biostimulator.

Why that word matters here

In our practice, “biostimulator” has a specific meaning. Dr. Rocco Piazza, MD, FACS is a board-certified plastic surgeon in Austin, Texas who places injectable collagen biostimulators as part of everyday practice. Those work by prompting your own fibroblasts to build collagen over months, rather than filling a space with volume.

A topical cannot do what an injection does, and we will not tell you otherwise. What this category claims to do is work along the same principle at the surface — supporting the skin’s own collagen synthesis nightly instead of all at once in a treatment room. That is a reasonable thing to want between visits, and it is the reason this bottle is on our shelf rather than a generic night cream.

Nightly, on clean skin. Introduce gradually if your skin is reactive. This is a maintenance product, not a post-procedure one — after a resurfacing treatment, wait until your care team clears you.

Kari, our medical aesthetician, ran the full Pavise regimen — including Bioadaptive Stress Repair nightly — for three months before we put it on the shelf. She went through an Austin summer on it.

What she reported at the end: a marked drop in pigmentation across the cheeks, no rosacea flares over the whole period, more even tone, and softer fine lines. That is one person on our own staff, not a clinical trial — and it is the reason this product is on the shelf at all. We do not stock something none of us has lived with.

Questions, answered

Is a topical biostimulator the same as Sculptra or Radiesse?

No, and it should not be sold to you that way. Injectable biostimulators are placed in the deeper tissue and prompt your own fibroblasts to build collagen over months, under a provider's hands. This is a nightly topical that works at the surface on the same underlying principle — supporting collagen synthesis a little at a time instead of all at once in a treatment room. That is a reasonable thing to want between visits. It is not a substitute for an injection.

Where does this sit if I already use a retinoid?

Both are evening products, so you choose or alternate. If you are tolerating a retinoid well, keep it and add this on your off nights. If your retinoid is causing flaking or stinging, swap to this nightly for two or three weeks, let the barrier settle, then reintroduce the retinoid less often. Do not start both in the same week — if something reacts you will not know which one caused it. If your retinoid is a prescription, raise it at your visit before layering.

How long before I see anything?

Pavise's own before-and-after documentation for redness and dyspigmentation is taken at four weeks. Collagen-directed change takes longer than surface hydration does, so judge this one over months rather than days. Hydration and calm usually show up first; firmness and tone are the slow part.

I get filler and Botox. Is there anything here that interferes?

Nothing in a topical reaches the plane where filler or a neuromodulator sits, so there is no interaction. The only rule is on the surface: leave injection sites alone for the rest of that day, then carry on as normal. What we do tell patients is that this is the product that protects what you spend on injectables — the texture and tone around a treated area is what makes a result read as good skin rather than good work.

Is it safe after a laser or in-office treatment?

Not immediately. Post-procedure skin follows a specific sequence and this is an active nightly therapy, so it comes back later rather than first. Confirm timing with your care team before restarting it after any treatment — the answer depends on what you had done and how deep it went.
