Exosome Serum: How It Works, How to Use It, and Where It Fits After Microneedling
What the evidence supports, how to use a serum at home, and why timing matters after a procedure.

Quick answer
An exosome serum is a cosmetic product containing extracellular vesicles, tiny packets cells use to signal one another. The strongest human evidence comes from exosomes used with microneedling, not on intact skin. At home, treat a serum as a well-formulated part of your routine, not a substitute for treatment. No exosome product is FDA approved.
- 3exosome sources: fibroblast, umbilical cord, bone marrow
- 1.2T+exosomes per bottle
- 30 mLtwice the size of a 15 mL serum
- $9.50per mL at today’s price of $285.00
What an exosome serum is, and is not
Exosomes are nano-sized vesicles that carry proteins, lipids and RNA between cells. The proposed mechanism: that cargo signals skin cells, including collagen-making fibroblasts. A reasonable hypothesis with early data, not settled science.
On a label, "exosome" can mean three different ingredients:
- Human cell-derived: from cultured umbilical cord, bone marrow, adipose or fibroblast cells.
- Platelet-derived: from processed human platelets, the category Plated Skin Science pioneered.
- Plant-derived or "exosome-like": plant vesicles or lab-made lipid particles. Cheaper, and not the same thing.
Two facts before you buy: the FDA states that there are currently no FDA-approved exosome products. And no serum contains living cells, so "stem cells on your face" is a red flag.
Where the evidence is: with a procedure vs. at home
- With microneedling: a 12-week randomized split-face trial (28 people, saline control) found more improvement in wrinkles, elasticity, hydration and pigmentation on the exosome side, with no serious adverse events (Park et al., 2023).
- Across studies: a 2026 systematic review found favorable early results in 8 studies, but only 171 patients and no follow-up beyond six months (Dhaliwal et al., 2026, via Dermatology Times).
- On intact skin: 56 people using a platelet-derived serum twice daily improved in redness, wrinkles and evenness at six weeks (Proffer et al., 2022). There was no control group, and participants also used sunscreen.
Independent reviewers note that penetration of intact skin by functional exosomes has not been proven in human trials (Dermatology Times, 2026).
| In-office, with microneedling or laser | At home, on intact skin | |
|---|---|---|
| Delivery | Treatment channels bypass the outer barrier | Must cross an intact barrier; how much does is unproven |
| Best evidence | Randomized split-face data, small studies | Uncontrolled studies, short follow-up |
| Role | Part of a treatment plan | Daily support between treatments |
Our position: a home serum is a reasonable addition to a good routine; bigger structural change comes from procedures. We say that even though we sell the serum.
Exosomes vs. PRP
- PRP is prepared from your own blood at the visit and has the longer research history.
- Exosomes need no blood draw, but come from donor or cultured cells and vary by manufacturer.
Independent commentary still ranks exosome evidence behind PRP. The right choice is a consultation question, not a shopping one.
How to choose an exosome serum
- What is the source? A brand that will not name it is telling you something.
- Per bottle or per mL? Exosome counts mean little without the volume.
- What else is in it? Vitamin C, growth factors or peptides change how it performs.
- Cost per mL? Bottles range from 15 to 30 mL or more, so sticker price misleads.
- Who sells it? A physician channel means someone accountable for post-procedure use.
- What does it claim? "Heals," "treats" or "cures" is a red flag.
Why we moved from Plated to INVO
Our patients used Plated Skin Science, and we stand by that. When we re-evaluated the category, we chose INVO Rejuvenating Complex Exosome Serum as its successor. The honest comparison:
| INVO Rejuvenating Complex | Plated INTENSE | |
|---|---|---|
| Exosome sources | 3: fibroblast, umbilical cord, bone marrow | 1: platelet |
| Volume | 30 mL | 15 mL |
| Exosomes per mL | About 40 billion | About 67 billion (denser per mL) |
| Cost per mL | About $9.50 | About $17.20 |
| Also contains | Vitamin C and growth factors | Peptides and antioxidants |
Plated INTENSE is denser per mL, and we will not tell you otherwise. INVO's case is breadth and value: three sources, twice the volume, roughly half the cost per mL.

“Plated served our patients well, and I’m not here to trash it. INVO earned its place because it does more in one step: three engineered cargo sources — umbilical, bone marrow and fibroblast — plus vitamin C and growth factors, in a bottle twice the size. It’s a true workhorse.”

How to use an exosome serum at home
- Cleanse gently (we use Pavise Gentle Amino Powerwash), then apply to dry skin before heavier creams.
- Follow the directions on amount and frequency. More is not better.
- Be consistent. Topicals take weeks, not days.
- Wear mineral sunscreen every morning. We pair exosome care with Pavise Dynamic Age Defense SPF 30, a 15% zinc oxide formula with a transparent finish.
- Add one new active at a time.
After microneedling or laser: timing matters
Freshly treated skin is open, so what goes on it is a clinical decision. Confirm timing with your care team.
- Your provider's aftercare sheet comes first. In-office exosomes are professional-use products, not a home serum.
- Hold a home serum with vitamin C until your provider clears you. INVO contains vitamin C, which can sting open channels.
- Hold retinoids and acids too. Gentle cleansing, bland moisturizer and sun avoidance come first; mineral sunscreen once you are cleared.
See our Laser Recovery Protocol for what we use after CO2, ContourTRL and other resurfacing.
How we use an exosome serum: three steps
-
Step 1
At home: AM + PM
Clean, dry skin, before moisturizer. Mineral SPF every morning.
-
Step 2
After microneedling: per your provider
Wait until you are cleared; INVO contains vitamin C. Confirm timing with your care team.
-
Step 3
Give it 8 weeks
Judge it after eight weeks of consistent twice-daily use. Individual results vary.
Who should skip it, or ask first
- Pregnant or breastfeeding: ask your physician before any new active.
- Active infection, open wound or a skin-condition flare: get assessed first.
- Known sensitivity to a listed ingredient, including vitamin C.
- Hoping a serum replaces a procedure: for deeper lines, laxity or scarring, talk with our clinical team.
Frequently asked questions
Do exosome serums actually work?
The best human data comes from exosomes used with microneedling, including a randomized split-face trial. Evidence on intact skin is earlier and mostly uncontrolled. A good serum is a useful, cosmetic part of a routine. Individual results vary.
Are exosomes FDA approved?
No. The FDA states that there are currently no FDA-approved exosome products. Topical exosome serums are sold as cosmetics and may not claim to treat, cure or prevent any condition.
Can I use an exosome serum after microneedling?
Follow your provider's aftercare and confirm timing with your care team. In-office exosomes are professional-use products. A home serum, especially one with vitamin C, waits until your provider clears you.
Is it better to get exosomes or PRP with microneedling?
PRP comes from your own blood and has a longer research history. Exosomes need no blood draw but come from donor or cultured cells, and none is FDA approved. The right choice is a consultation question.
What is a good alternative to Plated exosome serum?
We replaced Plated with INVO Rejuvenating Complex: three exosome sources instead of one, 30 mL instead of 15 mL, and roughly half the cost per mL. Plated INTENSE is denser per mL, so INVO's case is breadth and value, not concentration.
How long does it take to see results from an exosome serum?
Weeks of consistent use, not days. The most-cited topical study measured at six weeks; INVO's own before-and-after photos are at eight weeks of twice-daily use. Individual results vary.
